CONSULTATION
This will be an in-depth session for your provider to get to know you, find out your history and discover your goals. Several additional tests will be given to complement those given in the initial executive physical.
PLAN
Your physician will use your test results and goals to develop your plan. A customized wellness plan and exercise recommendations will be core components.
TOOL KIT
You will have access to information, tips and more to help you. A follow-up plan will also be set as accountability is a major factor for success and provides an opportunity to make plan adjustments.
REFERRAL
Should medical specialists be required as part of your patient care, your physician will make referrals to the best qualified physicians most capable of addressing your needs.
Unlimited messaging & Ongoing Patient Centered collaboration
Message your team on demand for medical advice, prescriptions, and wellness guidance. The team will be working in collaboration with the member’s primary care team.
What is the cost of membership?
Membership plan is priced at $300 per month. This cost covers the comprehensive care and unlimited messaging you receive from your doctor and health coach throughout the course of your yearlong membership.
An Assessment plan is a one-time fee of $250 for a 30-min session.
Lab work and specialty testing is not included in the cost of your membership.
When does my membership start?
Your digital membership begins the day you purchase it online. After signing up, you will have access to your private member personalized health ecosystem. You can schedule your first visit and access exclusive members-only discounts and perks from our partners.
What if I change my mind about my membership?
Assessment:
If you change your mind within 24 hours of signing up, we are happy to issue a full refund. Although we’d love to see you and let our care do the talking!
After 24 hours, you have 60 days from your purchase date to request a refund. If you have not scheduled your Assessment visit after 60 days, you will receive a full refund less a $50 service fee. If you've held either a medical or coaching visit, we cannot issue a refund.
Complete Plans:
If you change your mind within 48 hours of signing up, we are happy to issue a full refund. Although we’d love to see you and let our care do the talking!
If you cancel after 72 hours but before your first doctor’s visit, you will receive a refund less a $150 service fee.
After 72 hours, we offer a six month trial during which time you may cancel and receive a full refund minus fees associated with any services used according to the following schedule:
$500 - 60 minute doctor visit
$250 - 30 min doctor visit
$175 - health coach visit
After 6 months, you may cancel at any time for a no cancellation fee minus fees associated with any services used leading up to cancelled months.

What if I need to cancel or reschedule an appointment?
You have access any time to reschedule or cancel appointments in advance.
We do not charge for your first missed visit. After that, there is a 24-hour cancellation policy for all appointments with a doctor or health coach. Cancelling with less than 24 hours notice will result in a $50 charge to the credit card on file.
If you do not show up for an office, phone, or video visit without contacting our team, you will be charged $100. However, you will still be able to credit this towards a future rescheduled visit. In other words, the visit won't be forfeited.
Can I use FSA, HSA, or insurance?
Yes, you can use a FSA or HSA for your visits with us! Our care may be covered by your insurance provider as out-of-network medical care—many members get up to 100% reimbursed.
Why do you have a wait-list?
You may be put on a wait-list to ensure you can schedule a visit as soon as your membership begins. You will still be able to select your membership choice and enter your contact info. Then, we'll reach out when we have appointment availability. There is no cost for reserving your spot and you can opt out at any time. We will not charge your credit card until you are enrolled off of the wait-list. We will be in touch with you once that date rolls closer to schedule your initial visit.
When will It be available in my state?
We are rapidly expanding nationwide and opening membership to new states. If we are accepting new members in your state you can sign up directly for a membership.
